Chronic Disease Self-Management Program (CDSMP)

The workshop is led by trained facilitators, many of whom live with long-term conditions themselves. The workshops are available in-person through the University of Albany's Center for Excellence and Aging and Community Wellness Living Healthy program or online for a limited time through The National Council on Aging's website.
