| Chronic Disease Self-Management Program (CDSMP) |
| Nearly half of all adults live with one or more long-term health conditions and often struggle to find ways to manage their conditions. Stanford University has developed an evidenced-based, multi-week workshop that helps participants learn self-care techniques to reduce pain and anxiety and manage their symptoms. The workshop is led by trained facilitators, many of whom live with long-term conditions themselves. The workshops are available in-person through the University of Albany's Center for Excellence and Aging and Community Wellness Living Healthy program or online for a limited time through The National Council on Aging's website. |
