Albany County-NYCONNECTS
A Point of Entry access line for comprehensive long term care information, referrals, and/or assessments for the elderly, adults and children with disabilities, their family, or service providers at (518) 447-7177. P.O.E. is a collaboration between Albany County’s Department for Aging and Department of Social Services and works to improve or maintain the person’s health and daily function by providing access to affordable medical and non medical services.
Seniors: You may be eligible for the WRAP program which could help you lower your utility and fuel bills and help you stay warmer in winter and cooler for in the summer. The free program includes an energy audit and may include replacement or improvement to: insulation, weather stripping, old windows or doors etc. CLICK HERE for more info and eligibility
